An AI answer that was checked by a separate authority before it was released, with the decision and the reason written down at the time.
An AI output checked before it acts, by an authority independent of the model that produced it, whose determination binds execution, with a graded outcome and a record written at the time.

The answer passed every check and went out.
The answer was stopped and handed to a person before anything happened. This is the product working, not failing.
The answer was sent back with instructions to fix a specific problem, then checked again.
The answer was refused outright and never released.

Your firm's own rules, written so software can apply them: which regulations apply, what an answer must contain, and what must never go out.
A rule pack: a versioned set of conditions the governance authority applies to every decision.
The record written as the decision was made: the input, the checks, the outcome and the reason — chained together so nobody can quietly change it afterwards.
